The SOAP note template automatically pulls goals from the patient's Care Plan and populates them in the SOAP note for you. Each component of the note also provides areas for narrative text. Each goal in the SOAP note provides a field to mark the percentage achieved on that goal as well as if assistance was required. |

Use the Subjective area to denote the subject's current state as related to their diagnosis and past sessions and the Objective area to record progress on the subject's goals and how they performed on each. Specify quantifiable attributes related to each goal in this area of the SOAP note. ClinicSource™ inserts the goals from the care plan for you along with a dropdown from which to select the achievement percentage. The Assessment area can contain a summary of the session with regards to the subject's diagnosis and how they addressed their goals. Finally, use the Plan area to plot a course for the next therapy session and be sure to sign your SOAP note. |
